Companions Assistance Program – Overview

How the Program Works

  1. Connecticut residents can apply for a $125 voucher for general veterinary care. Please Note: The CVMF voucher program is NOT meant for emergency, urgent or next-day appointments.
  2. Applicants must be low-income or receiving some form of public assistance; proof of income is required.
  3. Vouchers are limited to one voucher per pet every twelve months. Each household can receive a voucher for up to two pets per year.
  4. If an applicant is approved, a voucher for $125 toward veterinary care will be mailed to the applicant along with a list of practices that accept CVMF vouchers.
  5. CVMF vouchers must be presented at the time of service to one of the veterinary practices that accepts them. The voucher will be applied to the bill.
  6. A voucher must be used within 60 days. If you have an unused voucher that is expiring, please contact us at president@petsafetyct.org.

How the Online Application Process Works

  1. Connecticut residents can apply on this website for a $125 voucher for general veterinary care.
  2. Applicants must complete the application in full.
  3. Applicants must upload proof of income.
  4. CVMF reviews applications and mails vouchers weekly.
  5. Approved vouchers are typically mailed with a list of participating practices within 5-10 business days.

Applicants can request a paper application by calling 860-635-7770.

A voucher is like cash. A CVMF voucher is a one-time use document to be used as a method of payment up to $125 at a participating veterinary practice.

CVMF vouchers are not for emergencies or urgent issues. If this is an emergency, please contact your veterinarian or bring your pet to an emergency hospital. You may apply for a CVMF voucher for a future or follow-up visit at a participating practice.

Your pet can receive one voucher every twelve months.

(Example: If Spot receives a voucher in June 2023, Spot will be eligible again in April 2024!)

You can get one voucher per pet, per year, for up to two pets per household.

Yes! You can get one voucher per pet, per year, for up to two pets per household.

(Example: You can receive one voucher for Spot and one voucher for Sparky).

No, vouchers must be used for two different pets.

(Example: If Spot and Sparky each receive a voucher, you cannot apply both vouchers to Spot’s visit).

Yes, expired vouchers can be reissued.
