The Connecticut Veterinary Medical Foundation (CVMF) is pleased to assist low-income pet owners. Through private and corporate donations along with CT Grants, we have secured a limited amount of funds. The distribution of these funds are limited and only 10 vouchers will be distributed each month. If you do apply and do not receive a voucher, your application will automatically roll into the following month.  We do hope to increase this amount over the next couple of months, but depends on further donations and/or grants received. 

• CVMF’s voucher program is designed to provide Connecticut resident pet owners who receive some form of public assistance with access to veterinary care. The application requires uploading proof of eligibility through income verification documents.
• Fill out the form below to apply for support. If eligibility criteria is met, CVMF provides a voucher certificate worth $125 of care that can be redeemed at a participating veterinary hospital. A list of hospitals that accept vouchers is sent along with the certificate. Please notify the hospital when you make your appointment that you will be using a voucher.
  • PLEASE NOTE: Due to the response and mailing of vouchers, this voucher is not intended to be used for emergency car If your pet is having an emergency, please bring them to an emergency hospital.
• Only two pets per household are eligible for assistance.  One voucher is available per pet per year.
• Vouchers are valid for 60 days.
  If you or someone else is not able to fill out the below form, please email president@petsafetyct.org or call 860-635-7770 for other methods of submission.
• Once the voucher is received, call the qualifying hospital of your choice to set up an appointment. On the day of the appointment, bring the voucher with you.
